Writing to a doctor via the Internet - inside view
In one form or another, the service “Internet access” was available in many regions 3-4 years ago. She worked on providing so-called. “Accessibility”, one of the most important parameters of the health facilities and the health care system as a whole. In 2012, the final year of the healthcare modernization program, the regional medical information systems were updated. The legislation in this area has also been updated - since 2013, the appointment for an appointment with a doctor via the Internet has been elevated to the category of public services.
Depending on how well we can provide it, the Government of the Russian Federation and the entire vertical below it will judge the effectiveness of the “mastering” of the money allocated for IT in medicine. Everything should be simple for the patient: I went to the State Services, chose the “record” in the section of the relevant department, indicated the necessary information - that's all: “Come to the clinic, and no queues!” ')
How it works? What kind of work do I need to do in IT medicine in medicine to ensure this “and everything”?
1. A bunch of 3 portals
There are 3 key portals, the interaction of which provides “recording via the Internet” - the RMIS portal, the ERD portal and the State Services portal. It so happened that a major modernization of health care did not give birth to a standard IIA, which could be used in all hospitals in all regions of this country. But the Ministry of Health managed to ensure that all regional IIAs from the uniform “zoo”, which is slightly less than 95% composed of FoxPro dinosaurs, become modular, meet modern requirements, and communicate with federal services and with each other according to some uniform protocols exchange and rules (enshrined in the "Recommendations" of the Ministry of Health of the Russian Federation). As a result, we have a bunch of 3 portals belonging to different departments:
1.1 Portal RMIS (regional medical system)
Managed by the Ministry of Health of the subject of the federation (region, region, republic). The main work on the implementation of RMIS is working with this portal. In each region it is its own, with us (Irkutsk Region) it is built by Rostelecom on the O7 platform
1.2 Portal Gosuslugi.ru
Managed by * known by whom * Ministry of Communications of the Russian Federation. All patients wishing to receive a public service (it is to this category that an appointment is made), regardless of the region, go to gosuslugi.ru , select the item “Request choose the section“ Services of the Ministry of Health of the Russian Federation ”and the item“ Receive applications (record) to the doctor “in the section Electronic services \ Ministry of Health of the Russian Federation and then follow the instructions of the system.
1.3 Portal FER (Federal Electronic Registry)
Managed by the Ministry of Health of the Russian Federation. “Communication” of RMIS and State Services takes place through an intermediary - FER . FER also duplicates the interfaces concerning the schedule of work of specialists and appointment. If necessary, you can add, delete or edit the settings of the record to specialists and register patients for an appointment without using the IIA

We create an employee, make him a resource, add a schedule ... Behind this long line is a lot of work on the transition from the “old” to the “new” IIA, which began in September 2012.
Each of the main menu items hides a separate module, which requires a very detailed provision of information, and what we put there will affect the adequate work of other modules.

On the eve of 2013, we opened a record for 2 specialists, just yesterday we expanded the list of available doctors to 8. Before the end of the year we have a difficult task - to transfer 100% of all types of records to electronic form, implement the EHR module and try to fly with all this submit annual reports and finally bury VFP6. It will be a difficult year ... Difficult, but interesting.
